Accident Alert - Cracked weld nuts on scissor lift steer axles result in potential pivot pin failure
Issue:
Genie Industries Inc. ('Genie') has received reports that cracked weld nuts on the steer axles of some GS-68 scissor lift machines have caused the pivot pin retaining fasteners to come loose.
In this situation, the pivot pin could fall out of its joint, causing the wheel to fall off the axle and the machine to tip over.
Action required:
- Locate all machines that fall within the serial number range 41905-43943.
- Immediately remove these machines from service.
- Contact Genie Australia Service Department at 00 61 7 3375 1660. Parts installation must take place before machines are returned to service.
- Fill out the attached completion form and fax to Genie's warranty department to verify that repairs have been completed.

Occupational health and safety regulations in Australia require that service bulletins be completed. It is the machine owner's responsibility to communicate this important information to service management and applicable managers.
